Position Summary

The Regional Palliative Care Network Director will report directly to the LHIN CEO and the RVP for Regional Palliative Care Network deliverables (or their assigned delegates). The RPCN Director will work with the Regional Clinical Co-Leads to achieve regional deliverables. The RPCN Director will collaborate with local partners to improve hospice palliative care across all patient populations, illness trajectories and health care settings.

Core Duties - Responsibilities

  • Work in close partnership with the Regional Clinical Co-Leads to achieve regional deliverables that are defined or outlined in the regional work plan.
  • Establish collaborative relationships with key stakeholders as identified through the regional work plan.
  • Work collaboratively with the Co-Chairs of the HNHB RPCN Committee, Vice President of Health System Strategy and Integration and the Director Regional Cancer Programs, Juravinski Centre and others to recommend innovative solutions to improve the system of care for patients with hospice palliative care needs and their families within HNHB LHIN.
  • Provide oversight and coordination of planning and information analysis activities required by the HNHB RPCN Committee and LHIN CEO and RVP CCO.
  • Prepare quarterly reports on behalf of the RPCN Committee for submission to the LHIN CEO and RVP, OPCN and others as requested
  • Maintain communication and collaboration among relevant health service providers to facilitate knowledge transfer and awareness of best practices
  • Provide project management support to the RPCN Committee
  • Ensure the development and implementation of any communication plan and engagement plan is completed in collaboration with the RPCN Committee LHIN CEO and RVP and/or assigned delegates.
  • Carry out all responsibilities in accordance with the Mission, Vision and Values of the HNHB LHIN and in alignment with OPCN direction and the Declaration of Partnership
  • Travel through HNHB LHIN geographic area and attend potential provincial meetings
  • Provide strong, visionary leadership at the regional level and serve as recognized leader in the region for hospice palliative care, demonstrating in-depth and knowledge of health system issues
  • Facilitate knowledge transfer of provincial hospice palliative initiatives and strategies to stakeholders throughout the region and act as a voice of the HNHB RPCN Committee (not the LHIN CEO or RVP CCP) at provincial tables.
  • Participate in provincial and regional committees as required



  • Completion of an annual regional work plan in collaboration with the Regional Clinical Lead. The annual regional work plan will be approved and supported by RPCN Committee and LHIN CEO and RVP CCO.
  • Report to the LHIN CEO and RVP (or assigned delegates) on a quarterly basis against annual deliverables
  • Engage with regional and OPCN leadership as required


  • Baccalaureate degree in health-related discipline, or relevant field and experience; Masters preferred
  • Knowledge of and had understanding or the palliative care system in HNHB LHIN
  • Thorough knowledge of The Declaration of Partnership
  • Knowledge of current and changing dynamics of the palliative and health care system
  • Demonstrated financial competence
  • Outstanding report writing, proposal development, and presentation skills
  • Self-directed; negotiation and conflict resolution skills
  • Understanding and experience with embedding quality improvement initiatives
  • 5-10 years of experience in systems planning
  • Planning and facilitation skills
