Position Summary

Provides leadership to assigned health system planning and integration initiatives, coordinating and conducting analysis to identify population health needs, service gaps, and opportunities for integrated service delivery across the HNHB LHIN.  Identifies and fosters LHIN-wide relationships among health service providers and related sectors that will result in improving the health of the population, enhancing the experience of care, and sustaining integrated service delivery.  An important part of the role will be to work with the LHIN’s Indigenous Health Network, First Nation Communities and Indigenous providers to improve access to and knowledge of culturally safe and appropriate care.

Core Duties - Responsibilities

  • Develops innovative approaches for provider partnerships and selects among alternative approaches to achieve planning and integration goals with a focus on Indigenous programs and projects.
  • Builds internal and external partnerships to advance planning, integration activities and initiatives in collaboration with HNHB LHIN Sub-Region Directors.
  • Conducts analysis and prioritizes opportunities for local system integration and coordination in alignment with strategic priorities.
  • Works with stakeholders and health service providers around integration, coordination of services and enhancements of health outcomes through agreements and planning projects.
  • Oversees all areas of project management including: development of project teams, planning and negotiations of resources, facilitating project development and implementation activities, identification and development of data sets, data collections and related analysis, monitoring project progress, stakeholder and community engagement, problem resolution and reviewing quality of work to ensure high standards are met by project team.
  • Participates on internal and external committees and work groups comprised of professional, administrative and clinical experts in order to promote and advance heath service plans and objectives of the HNHB LHIN.
  • Provides project leadership for the LHIN’s Indigenous portfolio including project management and support for the Indigenous Health Network, and attending LHIN Provincial Indigenous Lead calls and meetings as required.
  • Monitors and evaluates planning, engagement and integration outcomes.
  • Shares best practices on integration and coordination within and across LHINs and participates in provincial dialogue on integration methods and performance evaluation.
  • Additional portfolio specific responsibilities as required

  • Adheres to the LHIN’s patient safety policies and procedures



  • Post-secondary/Masters degree in health administration, business administration or relevant field.


  • Knowledge of and in-depth experience in working with First Nations communities, Indigenous health services and community organizations.
  • Demonstrated experience in planning process, analysis and evaluation of health care issues across multiple sectors.
  • An understanding of how health services are delivered in Ontario and a working knowledge of the role of the LHIN in planning, managing, evaluating and funding local health care services.
  • Demonstrated experience engaging and working effectively in partnership with a broad range of individuals and organizations of diverse backgrounds, preferably within the health care sector.
  • Strong interpersonal  skills  and  proven  ability  to  build  and  maintain  strategic  relationships  and partnerships with diverse health service providers.
  • Proven ability to build relationships and lead stakeholder engagement with First Nations, Inuit, and Metis populations.
  • Advanced project management knowledge and skills, and strong interpersonal and facilitation skills, along with proven ability to build and maintain strategic relationships with diverse health service providers.
  • Experience in analyzing and interpreting performance data, including financial data and quality indicators and evaluating program performance.
  • Demonstrated ability to thrive as a team player in a fast-paced collaborative team environment while managing a demanding workload effectively in an environment of many conflicting and competing deadlines.
  • Very strong verbal and written communications skills to effectively respond to the needs of the intended audience.
  • Experience with provincial reporting requirements and data sets.
  • Superior skills in the use of software including Microsoft word, spreadsheets and any other key software needed to perform duties, as well as knowledge of accessing internet to support research.
