  • Position Summary

    With joint accountability to the HHS Director of Community  & Population Health Services and the HNHB LHIN Director of Patient Flow, the Integrated Manager of Transitions will provide operational leadership in the effective transition/discharge of patients from the Juravinski Hospital (JH) to the community.

     

    The HHS/HNHB LHIN Integrated Manager of Transitions will directly lead identified JH/HHS staff whose function focuses on discharge planning and HNHB LHIN staff assigned to JH. She/he will be the link connecting the two organizations to embed the philosophy of Home First in all relevant processes supporting seamless and timely transitions between hospital and community.

     

    The HHS/HNHB LHIN Integrated Manager of Transitions will work collaboratively with:

    1. JH/HHS Clinical Managers responsible for the Emergency Department, In-patient units and their teams including Physicians 
    2. HNHB LHIN Home & Community Care Managers and their teams 
    3. Primary Care and,
    4. Community partners to achieve outcomes such as: a reduction in the length of time people are spending in the emergency department/hospital, the prevention of avoidable re- admissions to hospital, and a reduction in the number of people waiting in hospital beds for the right level of care.

    Qualifications

    • Regulated Health Care Professional in good standing with health professional regulatory body in Ontario
    • University degree in a health-related field, Master's level preparation an asset
    • Minimum 5 years management experience working within a unionized healthcare environment

    Skills & Abilities

    • Demonstrated ability to work independently and collaboratively with an interdisciplinary team, patients, families, primary care, community services required
    • Demonstrated leadership skills, including problem solving, critical thinking, conflict resolution and negotiation required
    • Excellent interpersonal, communication, organization and decision-making skills required
    • Demonstrated expertise in quality improvement and change management methodology required
    • Recent experience leading inter-professional team(s) through change at a system level
    • Expert knowledge and understanding of legislation governing health care delivery
    • Knowledge of direct care/case management models used in community health care
    • Demonstrated knowledge of local health and community services
    • Working knowledge of financial and statistical reporting
    • Excellent time management and stress management skills required
    • Fluency in English (verbal and written)
    • Computer literacy and basic proficiency required

    Other

    • Driver's licence and access to/use of personal vehicle for travel throughout HNHB LHIN area
    • Clear Criminal Record Check with Vulnerable Sector Search required
